Stay Covered With Benefits from Asereth

Asereth provides comprehensive and flexible benefit packages that are offered to full-time personnel.  The following is a brief synopsis of those benefits.  We hope this allows you to see "at a glance" all of the benefits offered by Asereth.

As a full-time employee of Asereth Medical Services, you are entitled to participate in the Asereth group medical, dental, vision, term-life, AD&D and long term disability plans.

Medical Insurance

Full time personnel (40 or 32 hours per week) are eligible for coverage after the first 90 days of employment.  Once you are enrolled, your benefits will become effective the first of the month following your eligibility.  Our medical insurance is provided by Blue Cross of California.

California Employees will have their choice of a PPO Plan (Prudent Buyer), Blue Cross HMO Plan (California Care) or Blue Cross HMO Power Select Plan.

Employees Residing in All Other States will be eligible to enroll in the Blue Cross Blue Card PPO Plan.

Dental Insurance

Full-time personnel are eligible for dental coverage after the first 90 days of employment.  Once you are enrolled, your benefits will become effective the first of the month following your eligibility.  Our Dental Insurance is provided by Ameritas Life Insurance Company.  This plan offers a Dental Preferred Provider Option (PPO) benefit.

Vision Insurance

Full-time personnel are eligible for vision coverage after the first 90 days, and the eligibility window is 30 days.  A minimum of 32 hours per weeek is required for eligibility.

If you are eligible for benefits and decide not to join - either some or all of the plans - you still must return the enrollment forms, which include areas for you to sign, stating that you are declining enrollment.
